Eligibility Requirements
1.	Employees must be actively at work and on the employer's payroll for a minimum of 20 hours per week.
2.	Employers and employees must be under age 65 in order to enroll.
3.	75% of all employees meeting requirements 1 and 2 above must be included.
4.	New eligible employees must be enrolled after 6 months of employment.
